Accident Reporting

An accident is an unplanned event (or sequence of events) that causes injury, illness, or property damage. A "near miss" is an unplanned event or sequence of events that have the potential to cause injury, illness, or property damage. A near miss is equally important to investigate because it indicates that something went wrong. It may only be sheer luck that someone wasn't injured - this time.

The supervisor and/or SEM will investigate the who, what, when, where, and how. The focus of any accident investigation is fact-finding, not faultfinding. The goal of an accident investigation is to find the cause(s) so that the supervisor can determine why it happened to help develop solutions to prevent recurrence.


Accident Reporting Procedures and Forms

All workplace injuries/illnesses must be promptly reported to SEM.

Emergency Medical Attention: Call 911 for ambulance to transport employee to St. Joseph Hospital or EMMC for medical treatment. St. Joseph Hospital is our preferred emergency healthcare provider for the Orono/Bangor area.

Non-Emergency Medical Attention: For injuries/illnesses that require non-emergency medical attention, contact Human Resources at 581-2360 to arrange an appointment. If unavailable, supervisors may call HealthWorks at 992-0099 to schedule an appointment. Medical care will be provided at HealthWorks in Bangor at 133 Corporate Drive, Suite 4.

Overnight Admission to a Hospital or Fatality: Immediately contact University of Maine Public Safety at 581-4040 to initiate further response.

Reporting Work Related Accidents or Illnesses:

For any illness/injury that requires medical attention or lost work time, contact Cambridge Integrated Services (UMaine Worker's Compensation program). If the treatment is first-aid only, you do not need to call Cambridge Integrated Services. The supervisor must promptly report work related injuries/illness by completing the following steps.

Injuries and Property Damage Sustained by Visitors, Students, or the General Public:

Please complete the Incident Report Form to report injuries or property damages sustained by visitors, students, or the general public alleged to have been caused by the University or occurring on University property or during a University activity or event (do not use this form for auto accidents or employee injuries).
